Chain of Custody Forms

A completed chain of custody form must accompany every sample submission. The COC documents sample identification, collection details, requested analyses, and the custody transfer from collector to laboratory.

Sampling Guidelines

Proper sample collection is critical to accurate results. Here are the basics:

General Sample Collection

  • Use only containers provided by the laboratory. Different tests require different containers and preservation methods.
  • Label all containers with site ID, date, time, and collector name before collection.
  • Fill containers to the appropriate level. Some tests require zero headspace; others do not.
  • Keep samples on ice (4°C) from collection through delivery to the lab.
  • Deliver or ship samples within the required hold time for each parameter.

Drinking Water Samples

  • Bacteriological samples must be collected in sterile containers with sodium thiosulfate (provided by the lab).
  • Run the tap for 2-3 minutes before collecting to flush the line.
  • Do not touch the inside of the container or cap.
  • Deliver to the lab within 30 hours of collection (bacteriological).
  • Lead and copper samples require specific first-draw protocols. We provide detailed instructions with the sample kit.

Wastewater Samples

  • Grab samples and composite samples have different collection requirements. Check your permit for which type is specified.
  • Some parameters require preservation in the field (e.g., pH adjustment for metals samples).
  • Fecal coliform samples must reach the lab within 6 hours of collection.
  • Record flow rate, weather conditions, and any operational notes on the COC.
